Our Duty

Well Managed Care acts as a liaison to families and local home health servicing agencies which includes Respite Care, Residential assisted living facilities and RN Case Managers.

We provide Community resource information and referrals. Providing support and guidance to clients and family members regarding hospital to home transitions. Advising on options for home care support services at home health care agencies and housing at assisted living facilities.

Types of Living Options

We are dedicated to assisting families with planning, healthcare resources and providing referral access for long term services and supports. Well Managed Care Resource Coordinators are Healthcare Professionals who are skilled in using a client-centered approach when locating home health services and finding resources for seniors and individuals with disabilities.

Assisted Living

Although assisted living facilities differ by state, services offered can include: Assistance with daily living activities (bathing, dressing, eating, toilet, etc.) Central dining programs that include three meals a day. Educational activities.

Memory Care

Memory care is a distinct form of long term care designed to meet the specific needs of a person with Alzheimer's disease, dementia or other types of memory problems.

Home Health Agencies

​Home Health Care: an agency or private organization that is primarily engaged in providing skilled or paraprofessionals home health care to individuals in out of hospital settings, such as private homes, group homes, and hospice.
